The posterior complex was exposed through posterior midline incision. Transpedicular screws were inserted, and one rod was placed temporarily to provide adequate stability during surgery. Circumferential debridement could be achieved through transpedicular approach, as shown in (A). Preparation of adequate length of allogenic fibular bone grafts for anterior column reconstruction (B). Introduction of the bone grafts through the routes between nerve roots without sacrificing any neurologic elements (C).
